I found the way it happens last night. I think you had a file uploaded with a _file_id.diz file in it (note the underscore) and Mystic wasGreat news :)
trying to process it for a file description. But it couldn't delete the file afterwards because it only actually deletes the standard filenames (file_id.diz and file_id.ans).
I've changed it last night so that it will only ever import
"file_id.diz" or "file_id.ans" files so the "_" prefixed file will never get processed in future which will correct the problem.
